Les cèl·lules de Langerhans, descrites per primera vegada pel patòleg alemany Paul Langerhans l'any 1868,[1] són cèl·lules dendrítiques, abundants a la zona suprabasal de l'epidermis i encarregades de la presentació d'antígens,[2] que contenen uns grans grànuls anomenats grànuls de Birbeck.[3] Fins a mitjans de la dècada de 1960 no va ser identificat el seu paper en la resposta immunitària innata contra microorganismes patògens o substàncies químiques nocives.[4] Anys més tard, fou descoberta la particular funció defensiva exercida per aquestes cèl·lules davant determinades infeccions víriques.[5] Normalment es troben a la pell[6] i als ganglis limfàtics,[7] però en cas d'histiocitosi se les pot trobar també en altres òrgans,[8] con ara els pulmons,[9] el cor,[10] el tracte gastrointestinal,[11] els genitals femenins,[12] l'ull,[13] el fetge,[14] el tim[15] o el cervell.[16] Estan involucrades en la patogènia del càncer bucal escatós,[17] de la psoriasi,[18] del granuloma eosinofílic solitari ulnar,[19] del granuloma eosinofílic orbitari,[20] de certs casos de malaltia periodontal[21] i d'hematopoesi extramedul·lar cutània neonatal,[22] de la dermatitis crònica,[23] de la dermatitis de contacte al·lèrgica,[24] del queratoacantoma,[25] de l'estomatitis aftosa recurrent,[26] de determinats fibromes odontogènics[27] del liquen esclerós vulvar,[28] del liquen pla oral i cutani[29] o de la leucèmia limfoblàstica aguda en alguns individus.[30]

Amb l'edat, el seu nombre i funcionalitat immunitària disminueixen,[31] fent la pell més procliu a patir infeccions i/o al desenvolupament de lesions neoproliferatives de gravetat diversa.[32] Algunes d'elles, molt singulars, són la variant no calcificada i rica en cèl·lules de Langerhans del tumor odontogènic epitelial calcificant[33] i la neoplàsia nodal de cèl·lules de Langerhans.[34] El sarcoma homònim és una neoplàsia maligna agressiva i poc habitual,[35] que de vegades es devenvolupa a partir d'una histiocitosi prèvia d'aquest determinat tipus cel·lular.[36] El carcinoma broncogènic associat a la histiocitosi X pulmonar és una malaltia inusual que afecta predominantment a adults fumadors.[37]
