La rabdomiòlisi és un trastorn en el qual el múscul esquelètic danyat es descompon ràpidament,[1] provocant greus trastorns electrolítics corporals.[2] Els seus símptomes inclouen dolors musculars, debilitat, vòmits i confusió.[3] Hi pot haver orina del color del te,[4] batecs cardíacs irregulars, síndrome compartimental produït per edema en els músculs afectats,[5] coagulació intravascular disseminada, hipotensió i xoc.[6] Alguns dels productes de la lisi muscular, com la mioglobina o la creatina-cinasa, són nocius per als ronyons i poden conduir a una insuficiència renal.[7]

El dany muscular és sovint el resultat d'una lesió per aixafament,[8] exercici intens,[9][10] medicaments[11][12][13] o abús de drogues.[14][15] Altres causes són infeccions,[16][17][18] lesions elèctriques o tèrmiques,[19] hipotiroïdisme greu,[20] polimiositis eosinofílica,[21] cop de calor,[22][23] hiponatrèmia,[24][25] intoxicació alcohòlica aguda,[26] abús de cocaïna[27] o d'altres psicoestimulants,[28] síndrome neurolèptica maligna,[29] lupus eritematós sistèmic,[30][31] consum d'esteroides anabòlics[32] o de certs suplements herbals sense control mèdic,[33] teràpies laxants,[34] immobilització perllongada,[35][36] falta de flux de sang a les extremitats,[37] síndrome de Cushing ectòpic,[38] síndrome de Paget-Schroetter,[39] síndrome de Stuve-Wiedemann,[40] síndrome de Sjögren,[41][42] consum de determinats animals tòxics,[43][44][45] enverinament per bolets[46][47][48] picades múltiples de vespa[49] o algunes mossegades de serps.[50][51] La rabdomiòlisi induïda per estatines[52] és un efecte advers iatrogènic poc comú que, excepcionalment, pot ser letal.[53] Algunes persones tenen trastorns hereditaris d'afeccions musculars que augmenten el risc de rabdomiòlisi, entre ells la malaltia de McArdle,[54][55] la malaltia de Tarui[56] o la distròfia muscular de Duchenne.[57] Hi ha casos en els quals l'origen d'aquest trastorn no pot ser identificat.[58] El diagnòstic es recolza en l'orina mioglobinúrica, en la que la tira reactiva d'orina detecta "sang" però que no conté glòbuls vermells en l'observació microscòpica del sediment d'orina.[59] Els exàmens de sang mostren una creatina-cinasa superior a 1.000 U/L. Quan la malaltia és molt greu el nivell d'aquest enzim pot superar les 700.000 U/L[60] o, excepcionalment, arribar a més d'un milió.[61] En alguns casos pot estar indicada la pràctica d'una biòpsia muscular per confirmar o descartar l'existència de patologies metabòliques subjacents.[62]

La base fonamental del tractament són grans quantitats de líquids intravenosos.[63] Altres tractaments poden incloure diàlisi o hemofiltració[64] en casos més greus.[65][66] Un cop establerta la producció d'orina, s'utilitzen habitualment el bicarbonat de sodi i el manitol, però no estan prou recolzats per l'evidència.[67] Els resultats són generalment bons si es tracta de forma precoç.[68][69] Les complicacions poden incloure potassi alt en sang,[70] calci en sang baix,[71] necròlisi epidèrmica,[72] coagulació intravascular disseminada i síndrome compartimental.[73]

La rabdomiòlisi es produeix en aproximadament 26.000 persones a l'any als Estats Units.[59] Tot i que el trastorn s'ha documentat al llarg de la història, la primera descripció moderna va ser després d'un terratrèmol a Messina el 1908.[74] Es van obtenir importants descobriments sobre el seu mecanisme durant el Blitz del Regne Unit de 1940-1941.[75][76] És un problema important per als ferits en terratrèmols,[77] esllavissades o enfonsaments d'edificis i els esforços dels socors per a aquests desastres sovint inclouen equips mèdics equipats per tractar els sobrevivents amb rabdomiòlisi.[75][78]
